Care Plan Development

Following an assessment, Elder Care Consulting has a care plan developed. The care plan is like a road map that identifies immediate and long term needs. The care plan is generally verbalized to the client/family. Elder Care Consulting also can provide a formal written plan, which can be shared with the client, caregivers and various agencies.
