What is a GCM?

A Geriatric Care Manager:

• Serves as the professional link between families and senior service resources.
• Works with families and aging individuals to help them cope with the emotional and physical stress of care-giving and to navigate the healthcare system.
• Provides a comprehensive in-home assessment of physical, cognitive, and functional needs.
• Conducts care planning assessments to identify problems and strengths.
• Screens, arranges and monitors in-home care or services.
• Reviews financial, legal, or medical issues and refers to the appropriate resources to assist in conserving assets.
• Acts as a liaison between families and long-distance caregivers.
• Offers guidance regarding alternative housing options including assisted living.
• Supports, educates and advocates for seniors and their families.
• The geriatric care manager works closely with the patient’s primary health care providers and family members to develop a plan to assist you and your family.
