
What is a GCM?
A Geriatric Care Manager:
- Serves as the professional link between families and senior service resources.
- Works with families and aging individuals to help them cope with the emotional and physical stress of care-giving and to navigate the healthcare system.
- Provides a comprehensive in-home assessment of physical, cognitive, and functional needs.
- Conducts care planning assessments to identify problems and strengths.
- Screens, arranges and monitors in-home care or services.
- Reviews financial, legal, or medical issues and refers to the appropriate resources to assist in conserving assets.
- Acts as a liaison between families and long-distance caregivers.
- Offers guidance regarding alternative housing options including assisted living.
- Supports, educates and advocates for seniors and their families.
- The geriatric care manager works closely with the patient’s primary health care providers and family members to develop a plan to assist you and your family.
