Lori O’Connor, an advanced practice geriatric nurse, established our parent company, Elder Care Consulting, LLC, in 2003. Elder Care Consulting is a geriatric care management company providing clinical assessments and ongoing care management to seniors and their families. In 2006, Elder Care Consulting determined a need to enhance services by the addition of Home Health Assistants. This resulted in the ability to provide a comprehensive model of care management. In January 2013, Lori created Elder Care Associates to manage in home services. Elder Care Services, which encompasses Elder Care Consulting and Associates, has grown to a staff of over 30, but continues to provide very individualized plans of care.
In an effort to broaden the reach of our mission to support elders in our community, we identified a need to provide clients with additional options for care in the community while remaining in their homes.
Our search led us to purchase a building in Tolland, Connecticut. The building currently hosts our Senior Day Center in the lower level. The center will serve as a safe, secure program for older adults while allowing engagement in meaningful, personalized activities. The second floor is being renovated to develop a resource center that will serve as a gathering place to educate and support families and caregivers about the process of aging.

Lori O’Connor holds a Masters degree in Nursing as a Geriatric Clinical Nurse Specialist. She is licensed as a Registered Nurse and holds national certifications by the National Association of Professional Care Managers. Lori has over 20 years experience practicing in a variety of senior organizations, home and community senior settings, building systems of care and advocating for older adults and their families.
Geriatric Care Managers
