Care Plan Development
Following an assessment, a care plan is developed. The care plan is like a road map that identifies immediate and long term needs. The care plan is generally verbalized to the client/family. We also can provide a formal written plan, which can be shared with the client, caregivers and various agencies.
Ongoing Case Management Includes:
- Recruitment and professional education to aides or home makers/companions.
- Coordination of care between multiple health care professionals.
- Assistance with navigating the complexity of the healthcare system.
- Support and advice on issues related to aging.
- Assistance with finding community support.
