Professional Staff

Lori O'Connor, Director and Owner
Director and Owner
Lori O’Connor holds a Masters in Nursing as a Geriatric Clinical Nurse Specialist. She is licensed as a Registered Nurse and Advanced Practice Registered Nurse. She holds national credentialing and certifications by the National Association of Professional Care Managers and the American Nurses Credentialing Center. Lori has over 20 years practicing in a variety of organizations and community settings building systems of care and advocating for older adults and their families.
Clinical Staff
Our clinical staff has extensive education and experience working with the geriatric population
Care Managers
Care Managers are either Registered Nurses or Social Workers with advanced degrees and have extensive experience with older adults and their families. They are responsible for the initial assessment and ongoing coordination.
Associate Geriatric Care Managers
(AGCM) are nurses or social workers. They are assigned to make visits to monitor clients, aides and to provide additional support. They report any findings directly to the Care Manager.
*Support Staff
Bookkeepers are responsible to help clients with bill paying, banking and sorting and filing paperwork and appropriate insurance information.
Home Makers
These individuals provide home making services, transportation to appointments, companionship and medication reminders.
Certified Nursing Assistants
These individuals provide all of the services a Home Maker does but they also can provide hands on personal care.
Moving Assistants
Elder Care Consulting provides transitional moves for seniors. We will help you find the best living arrangement and organize the entire move.
Music Therapy
Our Music Therapy program is designed for individuals who may be house bound. It is geared to clients who may have physical or memory impairment. Music has been shown to improve the mood and our therapists can develop a CD of the client’s favorite pieces.
Personal Chef Services
Shopping and preparing meals becomes even more difficult as we age. Take out menus and frozen dinners are frequently full of salt and fat. We can arrange for a personal chef to meet with you and your family. The Chef will then will develop a menu to your food preferences and health needs. They will shop, cook and freeze a months worth of dinners!
* Support staff is assigned based on the client’s plan of care. Two hours a month of Care Management are required to request these services.
